Welcome to the Intermountain Pet Hospital family! We are looking forward to getting to know you and your pet. You can rest assured that you have made the right choice for your pet’s veterinary care with us. It is our sincere hope that as we continue to build our relationship, you and your pet will soon become an integral part of our family.

When you come in for your first visit, please have your pet’s current shot record and a list of any concerns you may want to be addressed. We have the following forms available on our website. Please complete the online version and submit it directly to us. Or download the PDF version, fill it out, and bring it to your visit. When you come in, we will have you electronically sign any form requiring a signature. Filling out these forms prior to your visit not only saves you time when you’re in the office, but also allows you to focus on your pet while you are in the waiting room.

Records Transfer Form

This form gives your previous clinic permission to fax us a copy of your pet’s medical history. It’s helpful if this is sent to them before you come in so we can have it at the first visit.

Canine Risk Factors Form

Please complete this form to help us determine the correct preventive care protocol for your dog.

Prescription Refill Request Form

Use this form if your pet is already on medication and you are in need of a refill. Please allow 24 hours for us to fill the prescription.

Early Drop-Off Treatment Form

We understand the time constraints in your life and make every effort to accommodate your schedule. To meet your pet’s health needs, we are willing to allow patients to be dropped off. It’s important to note that this must be arranged prior to the visit. Please call first whenever possible, and leave this completed form with the receptionist or nurse when you leave your pet.
